Comparison of POC-CCA with Kato-Katz in Diagnosing Schistosoma mansoni Infection in a Pediatric L-Praziquantel Clinical Trial

چکیده
Introduction Traditionally Schistosoma mansoni infection is diagnosed by the Kato-Katz method. Thick smears from each stool sample are prepared on slides and eggs counted microscopically. Commercially available point-of-care circulating cathodic antigen (POC-CCA) cassette tests detect schistosomiasis antigens urine samples in 20 minutes. POC-CCA results qualitative or semi-quantitative: signal intensity an indicator of amount worm sample. Both methods were used a phase II trial investigating efficacy safety new pediatric formulations praziquantel (PZQ) among children ≤6 years (NCT02806232). This secondary analysis evaluated consistency between methods. Methods was to pre-screen for S. infection. Children with positive tested method, those (>1 egg/1 occurrence) enrolled. Participants (N=444) treated different doses PZQ. performed at 2–3 weeks after treatment evaluate drug efficacy. Cure rate (CR) defined as proportion participants negative result per POC-CCA, no Kato-Katz. Kappa statistic assess agreement cure status, Spearman correlation positivity egg counts evaluated. Sensitivity specificity calculated using reference standard. Results CR measured treatment, 52% [95% confidence interval (CI): 48%, 57%] across all arms except infants aged 3–12 months. 83% (95% CI: 79%, 87%). 0.16 0.09, 0.23), indicating that slightly better than chance. Relative Kato-Katz, POC-CCA’s sensitivity 70% 57%. coefficient 0.26 0.17, 0.34). Conclusion sensitive rapid diagnosing infection, but its performance requires further investigation young children.

Accuracy of Urine Circulating Cathodic Antigen Test for the Diagnosis of Schistosoma mansoni in Preschool-Aged Children before and after Treatment
BACKGROUND The Kato-Katz technique is widely used for the diagnosis of Schistosoma mansoni, but shows low sensitivity in light-intensity infections. We assessed the accuracy of a commercially available point-of-care circulating cathodic antigen (POC-CCA) cassette test for the diagnosis of S. mansoni in preschool-aged children before and after praziquantel administration. METHODOLOGY A 3-week ...
متن کاملSensitivity and Specificity of Multiple Kato-Katz Thick Smears and a Circulating Cathodic Antigen Test for Schistosoma mansoni Diagnosis Pre- and Post-repeated-Praziquantel Treatment
BACKGROUND Two Kato-Katz thick smears (Kato-Katzs) from a single stool are currently recommended for diagnosing Schistosoma mansoni infections to map areas for intervention. This 'gold standard' has low sensitivity at low infection intensities. The urine point-of-care circulating cathodic antigen test (POC-CCA) is potentially more sensitive but how accurately they detect S. mansoni after repeat...
متن کاملEfficacy and Safety of Praziquantel in Preschool-Aged Children in an Area Co-Endemic for Schistosoma mansoni and S. haematobium
BACKGROUND In sub-Saharan Africa the recommended strategy to control schistosomiasis is preventive chemotherapy. Emphasis is placed on school-aged children, but in high endemicity areas, preschool-aged children are also at risk, and hence might need treatment with praziquantel. Since a pediatric formulation (e.g., syrup) is not available outside of Egypt, crushed praziquantel tablets are used, ...
متن کاملAdditional Evaluation of the Point-of-Contact Circulating Cathodic Antigen Assay for Schistosoma mansoni Infection
Studies of the urine-based point-of-contact cathodic circulating antigen test (POC-CCA) in Schistosoma mansoni-endemic settings in Africa indicate it has good sensitivity in detecting infections, but in areas of low prevalence, the POC-CCA can be positive for persons who are egg-negative by Kato-Katz stool assays. We examined the POC-CCA assay for: (a) batch-to-batch stability; (b) intra-reader...
متن کاملEfficacy of oxamniquine and praziquantel in the treatment of Schistosoma mansoni infection: a controlled trial.
OBJECTIVE To evaluate the therapeutic efficacy of oxamniquine and praziquantel, the two most clinically important schistosomicide drugs, and to compare the accuracy of faecal examination with the accuracy of oogram in testing for Schistosoma mansoni infection. METHODS In a triple-masked and randomized controlled trial, 106 patients infected with S. mansoni were randomly allocated to one of th...
